We require a SUPPORT ASSISTANT 1\.\- Responsibilities of the support assistant regarding the accompanied person: • Conduct periodic visits to the person, both in stable situations and during crises. • Identify needs across all areas of their life, whether personal or related to their environment (e.g., clothing inventory, housing supervision). • Monitor and detect, when necessary, the correct administration of medication. • Provide emotional support and guidance to the person to ensure access to the human, personal, affective, and relational support they need. • Accompany them to medical services. • Accompany the person to disability degree assessments they must undergo (e.g., assessments at Evaluation and Guidance Teams and at the Catalan Institute of Medical Assessments, dependency assessments, etc.). • Accompany them to judicial procedures when necessary. • Accompany them to group activities scheduled by the organization. • Accompany them, if needed, to hospital emergency departments, assessing the situation and determining appropriate action during working hours. • Submit applications for technical aids, material resources, and medical or surgical examinations required by the person under guardianship. • Process the person’s personal documentation (ID card, renewals, transport cards, Social Security, health card, etc.). • Purchase personal items, food, clothing, and other goods to support and guide the person under guardianship. • Manage and justify expenses incurred in carrying out these duties. • Record all actions taken on behalf of the person under guardianship after each visit or intervention. • Ensure respect for relational customs and cultural diversity of the people served. • If applicable, arrange transfers for the person (e.g., changes of center, admission to residential facilities) together with their support reference person. 2\.\- Responsibilities regarding the support reference person and the entire team/organization: • Inform the support reference person and the center where the person is admitted to improve care and relay general medical information. • Coordinate periodically with the support reference person and the entire team. • Collaborate in developing, updating, and monitoring the person’s individualized care plan, evaluating their needs, choices, and preferences. • Participate in periodic team and organizational meetings. • Jointly develop the schedule of interventions and visits with the support reference person. • Attend and participate, if applicable, in supervision sessions. 3\.\- Responsibilities regarding the community and/or environment: • Promote activation, coordination, contact, and management of various natural supports, community-based supports, and specialized services across different domains (e.g., health, social services). • Raise awareness and inform the community about the guardianship service (its functions and responsibilities). * COMPLETED PRIMARY EDUCATION * Competencies / Knowledge: Individuals working as support assistants are expected to possess professional competencies specific to their role, based on technical and organizational skills acquired through training, as well as broader, transferable cross-cutting competencies applicable across various professional contexts—where cognitive and emotional competencies hold particular significance. Cognitive competencies refer to ‘the capacities and aptitudes a person must have to process information and obtain results’, whereas emotional competencies refer to ‘particularly significant characteristics, attitudes, or qualities a person must possess for effective performance of their tasks and responsibilities’.
