Are you passionate about community-based social work? We are seeking an energetic and committed professional to join the Home-Based Autonomy Support Service (SAPLL) team in Celrà (Gironès). Your purpose You will accompany individuals living alone — those with intellectual disabilities or social challenges stemming from mental illness — helping them gain autonomy in their daily lives. Your mission will be to provide them with tools, reinforce their self-confidence, and connect them with their surroundings so they can enjoy a full personal and social life. Direct accompaniment and support • You will design, implement, and monitor Individual Care and Intervention Plans (PIAI), with emphasis on the social domain. • You will work in tandem with an interdisciplinary team to help service users achieve their personal goals. • You will connect them with the community: educational, vocational, sociocultural, and leisure resources tailored to their interests and needs. • You will ensure they feel heard, satisfied, and experience well-being in their homes. Technical management with purpose • You will document interventions and rigorously and carefully record case file progress. • You will collaborate with the service’s psychologist to process benefits, subsidies, and social assistance that improve service users’ quality of life. • You will draft required reports and records so the service operates like clockwork. Networked work and environment • You will serve as the coordination bridge among reference professionals, health and social services, and the Person-Centered Care Service. • You will develop in-depth knowledge of each person’s family and social reality to deliver fully personalized support. • You will also serve as a trusted resource for families, informing and guiding them about local resources. * Experience: 1 year. Experience supporting people with functional diversity and mental disorders.
