Social Worker – Geriatric Residence
1\. Social assessment and support for residents
Conduct interviews and social assessments to understand residents' personal, family, and economic situations.
Assess social needs and develop individualized intervention plans.
Promote residents’ adaptation to the residence by providing emotional and social support.
2\. Support and counseling for families
Inform families about the residence’s operations and available assistance programs.
Accompany and guide families through the admission and adaptation process.
Facilitate communication among residents, families, and the professional team.
3\. Coordination and teamwork
Collaborate with the multidisciplinary team (physicians, nurses, psychologists, physiotherapists, occupational therapists, etc.).
Coordinate with social services, healthcare providers, and community organizations to ensure optimal care.
Manage administrative procedures (admissions, social benefits, allowances, pensions, etc.).
4\. Social activation and integration
Promote social and cultural activities to prevent isolation and encourage participation.
Strengthen residents’ integration into center life and foster their autonomy.
Identify situations of risk or vulnerability and take action to prevent social exclusion.
5\. Resource management and processing
Provide guidance on financial aid and benefits.
Process applications for complementary services (day centers, home care, etc.).
Collaborate with public administrations and organizations to improve service delivery.
* Minimum 1 year of experience; experience in long-term care sector
* Bachelor’s Degree in Social Work
* Temporary employment contract (6 months)
* Part-time schedule (15 hours \- weekly working hours)
