**Job Mission:**
Contribute to improving patient health through drug dispensing and/or pharmacotherapeutic follow-up via specialized pharmaceutical care that adds value to the healthcare process and promotes and enables the effective, safe, and efficient use of medications within a framework of comprehensive and continuous care.
**Responsibilities**:
* Drug procurement.
* Receiving drug orders from general hospital and sociosanitary pharmacy, verifying their accuracy. Placing them in the pharmacy storage, organizing them into the corresponding drawers for each patient.
* Periodic meetings with physicians to achieve coordinated and efficient drug prescribing, as well as to define rational storage and administration practices.
* Review and verification of delivery notes for drugs received from external pharmacies and general hospitals.
* Checking expiration dates of stocked medications and disposing of expired ones, managing their return to the external pharmacy's SIGRE point.
* Monitoring and adjusting minimum and maximum temperatures of medication refrigerators.
* Clinical follow-up regarding medication regimens per patient conducted by physicians and psychiatrists, aiming to ensure permanent updates of each resident’s medication.
* Preparation of rescue medication for treatment rooms, fulfilling requests received from nursing staff. Placing these drugs in the respective rooms.
* Ordering controlled substances from pharmacies, complying with authorizations from the relevant regional authority. Receiving and organizing them in the center's pharmacy storage.
* On-site review of patient medication drawers in trolleys across floors. Refilling missing items, correcting anomalies, updating drawers according to prescriptions issued by physicians or psychiatrists, and recording changes in GCR.
* Following physician-prescribed guidelines, placing orders for injectables, receiving them, and registering them in GCR per patient.
* Reviewing in GCR any incidents reported by nursing staff the previous day in the pharmacological field. Communicating with physicians or psychiatrists to assess necessary adjustments or modifications in each case.

**Requirements:**

* Degree or Bachelor's in Pharmacy
* FIR Specialty
* Minimum of 6 months of experience in a similar role, preferably in primary care, hospital, or elderly residence settings (desirable).
* Training courses related to the field, as well as complementary training promoting Equality, will be positively valued.
